Congresswoman Fletcher is hosting a Passport Fair with the Houston Passport Office to assist with passport applications and renewals. The Passport Fair will be located at the Bellaire Civic Center at 7008 South Rice Avenue on Saturday, March 8 from 8:30am - 4:30pm. You must register using the form below before your arrival.
You can book an appointment if:
- You’re applying for your first U.S. passport
- You’re applying for a child under 16
- Your previous passport was issued when you were under 16
- Your previous passport was lost, stolen, or damaged
- Your previous passport was issued more than 15 years ago
Please arrive on time for your appointment. All applicants, including minors, must appear in person.
What to Bring for Minors (Children Under 16)
Completed, printed application form DS-11
Completed, printed Form DS-3053 (Statement of Consent)
- For children under 16, both parents must be present or provide a notarized Form DS-3053 with a photocopy of the absent parent's ID.
- Sole legal authority documents may replace notarized consent.
Citizenship Evidence and Photocopy
- Certified U.S. Birth Certificate (Hospital Birth Certificate not acceptable), a copy of a Naturalization Certificate, or the original in addition to a copy of a Consular Report of Birth Abroad if applicable.
- Children under 16 must always submit an original birth certificate.
- Bring a photocopy of the original document.
Valid State Photo ID and Photocopy
- Copy of both parents’ I.D. needed (front and back).
- Both parents must be present or have a notarized copy of Form DS- 3053 (Statement of Consent) from the absent parent with a clear photocopy of the front and back of the identification presented to the notary. Sole legal authority documents may be substituted for notarized consent.
- If your form of ID is from out of state, please bring a second form of ID, such as a work ID, or insurance card with your current address.
- Bring a photocopy of the original document.
Recent Passport Photo
The photo must be 2x2, in color, with a white background, no glasses worn, and free of shadows.

- Passport Book: $100 (application) + $35 (execution)
- Passport Card: $15 (application) + $35 (execution)
- Expedited service: $60
- One-to-two-day delivery: $21.36

What to Bring for Adults (16 and older)
For applicants aged 16-17, at least one parent must be present. If neither parent can attend, proof of parental awareness in addition to your own photo ID is required.

- Certified U.S. birth certificate, Naturalization Certificate, Previous Passport, Original in addition to a copy of a Consular Report of Birth Abroad if applicable.
- Bring a photocopy of the original document.
Valid State Photo ID and Photocopy:
- Original state photo ID, and
- Bring a photocopy of the front and back of your ID (applicant or parent/guardian).
- If your form of ID is from out of state, please bring a second form of ID, such as a work ID, student ID, or insurance card with your current address.
- An applicant aged 16-17 will require at least one parent’s presence. If neither parent can be present, you must show proof of parental awareness in addition to your own photo ID.
- Examples of parental awareness: A check with a parent’s name on it, or a parent listed on the application as an emergency contact.

Passport Renewals
You are able to renew your passport online or by mail if the following applies:
- Your previous passport is undamaged and can be submitted with the application, and
- The passport was issued when the applicant was 16 years of age or older, and
- The passport was issued within the last 15 years, and
- The passport was issued in the applicant's current name or the applicant can provide legal documentation of a name change.
If these conditions are not met, applicants should use Form DS-11 to apply in person and must follow the same guidance presented as a first-time applicant.

Please note that the Houston Passport Office is unable to accept cash payments. The Houston Passport Office is able to accept debit and credit cards, as well as payment via check.
